    Staffing Pattern: 

    The Resident Care Services department at this assisted living community staffs through a primary care structure.  Each Caregiver will be charged with all of the personal care duties of their resident assignment.  Whenever possible each Caregiver will be assigned to the same resident group each day to promote continuity of care.  

    Responsibilities of the Caregiver:

    • Assist with activities of daily living, including passing medication as assigned, toileting, bathing, dressing, and laundry. 
    • Follow community protocol, licensing regulation and guidelines for both resident and employee safety.
    • Follow safety guidelines in the community, including proper lifting technique and universal precautions when providing care to the residents.
    • Follow the schedule of duties for the Caregiver, as well as the individual plan of care for each resident.
    • Monitor and log resident activity, food intake, functional status, psychosocial status, taking action as required to promote resident well-being.

Provides complete supervision of the medication room, pass techniques, documentation and supervision and provision of care related to medication in the community.

    The community has one Medication Aide per floor on each shift. 

    Responsibilities of the Medication Aide:

    • Resident charts. Keeping documentation current (Community forms, licensing documentation, physician orders, incident reports., etc.)
    • Communicate resident status changes.
    • Ensure all medication documentation is current and correct, including medication administration forms, physician orders, change of dosages, written orders to confirm telephone orders, etc.
    • Ensure the medication room is completely stocked with all required continuous, PRN, Over-the-Counter (OTC), and other medications as ordered by the physician.

    Description of Position: 

    The Quality Care Manager is responsible for the overall quality of care provided to the residents. Oversees the medicine management, staff evaluations, and resident care in the community. Ensures that the care staff understands the standard of care as defined by Aspen Creek and that those standards are met.

    Responsibilities:

    Med Management

    • Manage residents’ medications in accordance with family, physician and pharmacy.
    • Process new medications as they are prescribed and filled
    • Update any medication changes or medication discontinuations in EHR
    • Under the direction of the registered nurse, manage medication, oversees the administration of medications to residents, order medication from pharmacy and maintain records as dictated in the policies and procedures manual. 
    • Assures that medications are properly stored, medication carts/cupboards are cleaned and the medication room is organized according to standards.

    Evaluations

    • Provides direct supervision of resident med tech staff, which includes monitoring job performance, conducting associate performance evaluations, coaching, conducting and documenting corrective action as needed. 

    Resident Care

    Description of Position:

    The Director of Clinical Care works as a liaison between residents, resident families, and staff. The Clinical Director duties include overseeing resident care plans and medication management, education/training, and coordinating all medical needs of the residents.  The Clinical Director must be an RN.

    Staffing Pattern:

    The community has one Clinical Director position.  This is the chief supervisory position for the individuals who provide primary care to their resident assignment.

    Responsibilities of the Clinical Director

    Care Plans and Medication Management & Coordination

    • Works in conjunction with the Quality Care Manager to review and approve schedules and assignments for the med-tech staff.
    • Ensure proper medical staff-to-resident ratios are met at all times and are within the budgetary requirements.
    • Conduct 30-day and 4-month re-assessments for each resident.
    • Ensure that the delivery of medical services are consistent with prepared care plans.
    • Ensure that care plans are scored or categorized appropriately so residents can be billed appropriately. Work with the controller to ensure that the rates that residents are charged are aligned with care plan scoring or categorization.
    • Coordinate and communicate with staff and other departments to ensure successful resident outcomes.
    • Supervise resident medications, working with and supervising Medication Aides.
    • Coordinate care planning with home health agencies on site, working within community policy.
    • Coordinate admissions with the Executive Director and Assistant Executive Director including supervising move-ins to be sure accommodations are as desired, new residents are welcomed to the community, and care is properly, seamlessly, and immediately implemented.
    • Participate in and/or conduct resident admission assessments and interviews.
    • Arrange for resident special needs involving other departments, verifying follow through.

    • Follow safety guidelines in the community, including proper lifting technique and universal precautions when providing care to the residents.
    • Follow the schedule of duties for the Caregiver, as well as the individual plan of care for each resident.
    • Monitor and log resident activity, food intake, functional status, psychosocial status, taking action as required to promote resident well-being.
    • Report status change immediately to the supervisor.
    • Act immediately on any resident crisis, following protocol and basic first aid training.
    • Document resident status change, including but not limited to, physical change, reaction to medication, psychosocial status change.
    • Maintain respectful, professional boundaries with residents. 
    • In the event all assigned duties cannot be completed, ask for assistance and report to the personal care coordinator.
    • Any other assignments made by your direct supervisor or administrator.                    
    • Promote open communication between health care professionals, families, residents, and staff.
    • Adhere to guidelines in the employee handbook including dress code, conduct, scheduling, etc.
    • Assist residents with room cleaning and perform various facility cleaning duties as identified. 
    • Assist residents with attending meals and activities. Encourage participation in social activities and attend activities with residents when time permits. 
    • Communicate vital information with colleagues between shifts to ensure continuity of care.
