Description of Position:
Responsible for the daily cleaning and laundry requirements as directed by the Housekeeping Supervisor.
Responsibilities of the Housekeeping Staff:
- Daily cleaning of resident rooms and bathrooms, laundry services, and other required cleaning within the facility are maintained to company standards.
- Ensure walkways are swept and clear of hazards. This includes removing snow and ice when needed.
- Report maintenance repairs needed.
- Weekly cleaning of the administrative offices.
- Maintain the proactive cleaning list (deep cleaning items) as detailed in the proactive maintenance log.
- Maintain organization and cleanliness of laundry rooms and maintenance closets.
- Act as first responder in emergency situations if Caregivers are not immediately present.
- Other duties as directed by the Housekeeping Supervisor.

Assists in the preparation, cooking, delivery, and clean-up of each meal. Ensure that state food health and safety regulations are met. Participate in regular cleaning activities to maintain an orderly kitchen
Responsibilities of the Dietary Staff:
- Perform tasks relating to food and beverage delivery as assigned by the Director of Dietary Services.
- Ensure that the kitchen is kept clean, orderly, and sanitized.
- Follow all rules and mandates for running a commercial kitchen and ensure the kitchen remains compliant at all times.
- Work with other staff such as the Caregivers to assure residents receive their meal in a timely manner.
- Assist with inventory and organization of all food and supplies.
- Assist with dating all food items that go into refrigeration units.

Life Enrichment Assistant

Description of Position:
The Activity Assistant will ensure residents at Aspen Creek have a variety of activities to keep them engaged and entertained to the extent they desire.
Responsibilities of Housekeeping Supervisor:
- Work with residents to identify activities they are interested in participating.
- Scheduling, obtaining supplies and leading activities requested.
- Engage reluctant residents to encourage participation.
- Engage in 1:1 activity with residents who prefer not to join group activities.
- Document activities and share through social media accounts with permission of residents.
- Be aware of community events that Aspen Creek may participate.
- Help maintain an activity calendar.
- Ensure that there is vibrant life throughout the Aspen Creek community.
- Lead the department in the absence of the director.
- Other duties as assigned.
Caregiver
CAREGIVER
Department: Clinical Care Services
Reports Directly to: Director of Clinical Services
Description of Position:
Provides direct personal care and supervision to the residents at the community. Promotes resident well-being and satisfaction through support with activities of daily living. Communicates with other departments to ensure resident needs are met.
Staffing Pattern:
The Resident Care Services department at this assisted living community staffs through a primary care structure. Each Caregiver will be charged with all of the personal care duties of their resident assignment. Whenever possible each Caregiver will be assigned to the same resident group each day to promote continuity of care.
Responsibilities of the Caregiver:
- Assist with activities of daily living, including passing medication as assigned, toileting, bathing, dressing, and laundry.
- Follow community protocol, licensing regulation and guidelines for both resident and employee safety.
- Follow safety guidelines in the community, including proper lifting technique and universal precautions when providing care to the residents.
- Follow the schedule of duties for the Caregiver, as well as the individual plan of care for each resident.
- Monitor and log resident activity, food intake, functional status, psychosocial status, taking action as required to promote resident well-being.
- Report status change immediately to the supervisor.
- Act immediately on any resident crisis, following protocol and basic first aid training.
- Document resident status change, including but not limited to, physical change, reaction to medication, psychosocial status change.
- Maintain respectful, professional boundaries with residents.
- In the event all assigned duties cannot be completed, ask for assistance and report to the personal care coordinator.
- Any other assignments made by your direct supervisor or administrator.
- Promote open communication between health care professionals, families, residents, and staff.

Coordinates resident care related to medications by working with all departments and administrative staff to provide for resident needs with continuity and an adherence to the scope of practice and licensure for the community. Provides complete supervision of the medication room, pass techniques, documentation and supervision and provision of care related to medication in the community.
Staffing Pattern:
The community has one Medication Aide per floor on each shift.
Responsibilities of the Medication Aide:
- Resident charts. Keeping documentation current (Community forms, licensing documentation, physician orders, incident reports., etc.)
- Communicate resident status changes.
- Ensure all medication documentation is current and correct, including medication administration forms, physician orders, change of dosages, written orders to confirm telephone orders, etc.
- Ensure the medication room is completely stocked with all required continuous, PRN, Over-the-Counter (OTC), and other medications as ordered by the physician.
- Coordinate medication orders and deliveries with pharmacies
- Communicate with physicians and other healthcare providers as needed.
- Monitor Psychotropic med use is congruent with physician orders and ensuring resident behaviors actually warrant the use of medication.
- Control medication room access and key assignment.

Description of Position:
The Quality Care Manager is responsible for the overall quality of care provided to the residents. Oversees the medicine management, staff evaluations, and resident care in the community. Ensures that the care staff understands the standard of care as defined by Aspen Creek and that those standards are met.
Responsibilities:
Med Management
- Manage residents’ medications in accordance with family, physician and pharmacy.
- Process new medications as they are prescribed and filled
- Update any medication changes or medication discontinuations in EHR
- Under the direction of the registered nurse, manage medication, oversees the administration of medications to residents, order medication from pharmacy and maintain records as dictated in the policies and procedures manual.
- Assures that medications are properly stored, medication carts/cupboards are cleaned and the medication room is organized according to standards.
Evaluations
- Provides direct supervision of resident med tech staff, which includes monitoring job performance, conducting associate performance evaluations, coaching, conducting and documenting corrective action as needed.
Resident Care
- Provides direct supervision of resident care staff, which includes monitoring job performance, conducting associate performance evaluations, coaching, conducting and documenting corrective action as needed.
- Assist with initial assessments for new potential residents, as directed by the clinical director (RN does the initial assessment, but can delegate to Quality director).
- Participate in interdisciplinary team meetings
